eSanjeevani Programme (Online OPD)

eSanjeevani – a doctor to doctor telemedicine system is being deployed nationally for the Ministry of Health & Family Welfare at 155,000 Health and Wellness Centers under Ayushman Bharat Scheme of Government of India.

 

eSanjeevani', is a web-based comprehensive telemedicine solution. It is modeled on ‘Sanjeevani’ CDAC Mohali’s' flagship integrated telemedicine solution. 'eSanjeevani' extends the reach of specialized healthcare services to masses in both rural areas and isolated communities. Besides enhancing quality of medical services, addressing issues pertaining to uneven distribution and shortage of infrastructural as well as human resources, eSanjeevani also aims to make healthcare services equitable by bridging the digital divide that exists between the urban vs. rural, rich vs. poor etc. eSanjeevani can also be used to provide medical education to interns , people across various Common Service Centers (CSCs), etc.

Technology:

eSanjeevani is a platform independent, browser-based application facilitating both doctor-to-doctor and patient-to-doctor tele-consultations. It has a user friendly GUI and has been developed on .Net technology using 3.5 framework with MS-SQL as the database. It provides the ease of accessing the health records at the comforts of one's home. The application is based on invite-system which restricts it to the actual beneficiaries of the application. It has a user-friendly interface which facilitates both tech savvy and novice doctors/users in the rural and urban environment to access the application. The application also has advanced degree of security against unwanted elements.

Launched by Union Minister

eSanjeevani, was launched by the Honorable Minister of State (Communications and Information Technology) Sh. Sachin Pilot at Department of Information Technology, New Delhi on 16th June 2009. Sh. Sachin Pilot highlighted the need to have a telemedicine network at rural areas and how beneficial eSanjeevani can be in providing primary healthcare across the Indian region.

 

Key Features

MIS-based application

eSanjeevani has strong administrative abilities to manage the users of the application. Regular alerts in terms of memory usage, invites, blocked users etc. help keep the users updated. It has been designed as a highly customizable and flexible application for the users to choose from the varied specialties such as tele-ophthalmology, tele-cardiology etc. and functionalities. The users can choose the desired specialists and hospitals for tele-consultation as per their requirements. Various logs are maintained to record user activity.

Comprehensive Electronic Medical Record (EMR)

eSanjeevani's EMR includes demographic and other patient data like clinical examination reports, physical examination reports along with patient's medical history, family's medical history, etc. eSanjeevani also enables import and export of complete patient record including images, files etc.

Tele-Consultation

eSanjeevani provides tele-consultation facility using both 'store and forward' and 'real-time' mode. In a 'store and forward' mode, the users can choose the doctors in the specialized area for the diagnosis and send the selected/ entire EMR for consultation. For 'real-time' consultation, video conferencing facility is provided to the user to connect to the doctor.

Video Conferencing

The web based video conferencing enables doctors to have a video/audio conversation with the consulting medical experts at remote end. Doctors can also exchange clinical information along with the radiographs for a detailed discussion. Online text based chat facility has also been provisioned.

 

National Teleconsultation Service :

National Teleconsultation Service of Ministry of Health and Family Welfare is first of its kind onlineOPD service offered by a country government to its citizens. National Teleconsultation Service aims to provide healthcare services to patients in their homes. Safe & structured video based clinical consultations between a doctor in a hospital and a patient in the confines of his home are being enabled.

eSanjeevaniOPD :

eSanjeevaniOPD – Stay Home OPD has been developed by Centre for Development of Advanced Computing (C-DAC) in Mohali. Salient features of this citizen friendly web-based National Teleconsultation Service (eSanjeevaniOPD) are:

  1. Patient registration
  2. Token Generation
  3. Queue Management
  4. Audio-Video Consultation with a Doctor
  5. ePrescription
  6. SMS/Email Notifications
  7. Serviced by State’s Doctors
  8. Free Service
  9. Fully configurable (no. of daily slots, no. of doctors/clinics, waiting room slots, consultation time limit etc).

eSanjeevaniOPD is based on eSanjeevani - Government of India's flagship telemedicine technology developed by Centre for Development of Advanced Computing (Mohali). eSanjeevani - a doctor to doctor telemedicine system is being deployed nationally for the Miinistry of Health & Family Welfare at 155,000 Health and Wellness Centres under Ayushman Bharat Scheme of Government of India.

Needed to Uses of eSanjeevani OPD

  • Users must have a laptop or a personal computer with a web camera connected with Internet (1Mbps)
  • eSanjeevaniOPD to be used in Chrome browser.
  • If you are using MS Windows operating system, please ensure you have Windows 8.0 or later in your laptop/PC.
  • A working mobile phone to send/receive OTPs by SMS

 

FLOW:
 

Registration:

-User verifies his/her mobile no. using OTP
-Fills Patient Registration Form
-eSanjeevaniOPD assigns a Patient ID
Token:
-User requests a token for consultation
-Uploads health records, if any
-User receives Patient ID & Token through SMS
 

Login:

-Closer to the turn eSanjeevaniOPD sends a SMS notification asking user to login
-User logs in using Patient ID
-Patient enters the clinic and is placed at the end of the existing queue. If there is no queue you will be placed at serial no. 1

Wait:

-eSanjeevani OPD assigns a doctor to the patient (time interval depends on the length of the queue)
-As the doctor is assigned to the patient “CALL NOW” button gets activated
-User is required to click “CALL NOW” button within 120 seconds*
-Upon clicking “CALL NOW” within 10 seconds the doctor shows up in video
 

Consultation:

-Patient consults the doctor
-During the consultation doctor has an access to the patient’s health records (if uploaded)
 

ePrescription:

-During the consultation, doctor prepares an electronic prescription (ePrescription)
-At the end of the consultation doctor sends the ePrescription and closes the call
-ePrescription shows up on patient’s end.
-Patient logs out after saving/printing the received ePrescription

Other provisions:

  1. eSanjeevaniOPD is yet a web-application, however, it’s development has been based on responsive web design approach. Hence eSanjeevaniOPD is expected to respond to the user’s behaviour and environment based on screen-size, platform and orientation. It is possible to use eSanjeevaniOPD on large screen tablets and smartphones.
  2. For a smooth full-motion video consultation experience internet speed of at least 1Mbps is recommended.
  3. Once registered patient’s name, age, gender, patient ID cannot be altered, however, patient can update email / mobile number and address
  4. Every patient is assigned a unique 16 digit patient ID, it is generated using Luhn Algorithm (mod 10 algorithm).
  5. Every consultation will be assigned a unique consultation number.
  6. Patients residing in a particular State will get connected with one of doctors on the panel of doctors set up by the corresponding State’s health department.
  7. If (at xi) patient misses to click “CALL NOW” in 120 seconds, eSanjeevaniOPD will move this patient few slots lower in waiting room queue and the patient next in queue will take the turn.
  8. All tokens (used & unused) will expire at the end of the day.
  9. For using the system again the user will need to follow same steps, registration form will show up auto filled with details existing in eSanjeevaniOPD, hence the user will upload health records if any and then generate token.
